Submission Requirements 		 

A complete submission should include

 
  1. Hard copies of the
    • submission form (Annex E)
    • completed nomination form (Annex B) endorsed by the Principal
    • letter of undertaking signed by the nominee (Annex F)

  2. A CD ROM containing soft copies of the following
    • the nomination form (Annex B)
    • the lesson plan (Annex C)
    • the lesson review (Annex D)
    • lesson materials/resources (worksheets, templates etc)
Note:  
Lesson plan should not have been submitted before for other competitions or awards.  
The lesson need not be limited to implementation within the classroom.  
There must be clear and accurate acknowledgement of the source(s) of materials used if they are not original creations  
Incomplete submissions will be disqualified. Judges’ decision is final.  
   
Closing date  
8 July 2005 (Fri), 6pm
